Severe accident (SA) like core meltdown had not been considered to occur in Japan because we believed to have taken perfect measures for 1st~3rd layers of the defense in depth(DiD). The direct cause of Fukushima accident was loss of all AC power by an unexpected huge tsunami, but the fundamental cause was insufficient measures for the 4th&5th layers of DiD.
